RISK MANAGEMENT INFORMATION NORTH TEXAS REQUIRES THAT A RISK MANAGEMENT DISCLOSER FORM, BE FILLED OUT ONCE A YEAR North Texas will be using GotSoccer as the exclusive risk management software. Once you are registered in gotsoccer: You will be sent an email from support@gotsport.com, with a user name and link to access the login page. You will have to login and click on "Can't remember password", a password will be sent to you immediately. It is mandatory that everyone completes the background check. The system will not allow me to add any adult to the roster until the background check has been completed and approved.

b. Required paperwork must be brought to office to receive
     Spring 2012 Roster

Teams with NO Changes:
a. Complete GotSoccer Online Registration

Teams With NO Changes Only MAY PRINT TEAM SPRING 2012 ROSTER 1. If you have "No Changes" to Roster. 2. If "Club" writes check for Registration. "Once Registration is Approved" You may Print your Spring 2011 Roster. Login to GotSoccer Team Account Click on Team Profile>Document> Print Spring 2011 Roster

UPDATE YOUR GOTSOCCER PLAYER LIST INSTRUCTIONS

1.  Players Not Returning:  - Delete
      a. If leaving team - Release signed by North Texas is required
      b. If transferring to another team - Release signed by North Texas approving           transfer is required

2.  Returning Players: Update information (changes-address-phone#-email)
    
3.  New Players:  Add player in GotSoccer Team Account.
     a. All required paper work must be turned into the office before player can be          added to Official  Roster.

4.  Coach/Manager: Update information. (changes-address-phone#-email)
     a. If New - Background Check is required before being added to team
